Shining Star Adult Day Health Care Center

Adult Day Health Care, designed to serve medically, physically and/or cognitively impaired adults, provides a structured day program which includes nursing, social services, restorative therapies such as physical, occupational, and speech therapies, dietitian and mental health therapist. Meals, and transportation are also provided in a warm, caring, and professional environment. The program focuses on person-centered care for each person based on the participant's plan of care.

Medical Nursing Coordination Medical Social Work 

• Supportive counseling with community resource referrals 

PROFESSIONAL REHABILITATION SERVICES:

• Registered Dietitian 

• Psychiatric/ Psychological Services 

• Physical Therapy 

• Occupational Therapy 

• Speech Therapy 

SKILLED NURSING SERVICES 

• Registered Nurses on-site daily to evaluate and provide medical and nursing care coordination. 

THERAPEUTIC ACTIVITIES 

• Designed to stimulate, educate, and increase socialization with participants. 

TRANSPORTATION SERVICES BREAKFAST & LUNCH

Individuals may be eligible if they meet all of the following criteria: 

1. Have a medical condition that requires treatment or rehabilitative services prescribed by a physician. 

2. A Physical and or mental impairment that limits activities of daily living, but not so serious as to require 24-hour institutional care. 

3. A reasonable expectation that preventative services will maintain or improve the present level of functioning. 

4. A high potential for further deterioration and probable institutionalization if ADHC services were not available.

For CBAS eligible participants, program services may be covered by the following insurances: 

• MOST MAJOR MEDI-CAL MANAGED CARE PLANS
• LONG-TERM CARE INSURANCE
• PRIVATE PAY
